Permission Form

The purpose of this form is to inform you about the excursion and to seek your support and permission for your child to participate. This information may be shared as necessary with adults supervising the excursion.

Date of event: 10am-2pm; Thursday July 7, 2022

Excursion details: We’ve been invited to attend two (2) basketball games at the Canada Basketball’s Globl Jam event at the Mattamy Centre (Toronto Metropolitan University (TMU)).

Please note: Participants need to bring their own public transit fare. If you aren’t able to provide public transit fae, please let us know and arrangements will be made.
